As SWEDISH CARE SAĞLIK HIZMETLERI ANONIM ŞIRKETI (hereinafter it shall be referred to as “Data Controller” or “Company“), we hereby declare that our purpose in preparing this Clarification Notice is to inform you in the most clear and transparent manner about the ways your personal data are collected, the purposes of processing such personal data, the persons with whom they are shared, the legal reasons and your rights pursuant to Article No 10 of the Law No 6698 on the Protection of Personal Data (hereinafter it shall be referred to as the “Law“). In order to ensure that your personal data is processed, stored and shared in compliance with the laws and to protect your privacy, our Company takes the highest possible security measures.
Our company was established and is operating under the laws of the Republic of Turkey within the scope of the Regulation on the Provision of Remote Health Services published by the Ministry of Health, in order to provide services in the field of non-residential practice activities related to specialized medicine provided in private medical centres (excluding hospital and polyclinic activities and paramedical activities of midwives, nurses and physiotherapists).

2. Your Personal Data Processed by Our Company After You Fill In the “Contact Us” Form in the “CONTACT US” Section on Our Website
After you fill in the Contact Us Form in the “CONTACT US” section on our website, pursuant to Article No 5/2/(c) of the Law, based on the legal ground that “It is necessary to process personal data belonging to the parties to the contract, provided that it is directly related to the establishment or performance of a contract”, the following data (“Personal Data”) are processed by our COMPANY in whole or in part by automated methods (electronic forms):
- Identity Information, (Name, Surname)
- Contact Information (Phone Number, E-Mail Address)
- Employment Information (Company Name, Title)
- Other information regarding your claims (If the claim you send to us through the “Contact Us” section on our website is specified in the “Subject” section, your personal data therein)
These personal data of yours are processed for the following purposes and for the periods required by these purposes:
- Evaluation and follow-up of your claims,
- Communicating with you regarding your claims,
In case your Personal Data are subject to a legal dispute, we may process your data for the purpose of “Using this data as Evidence in Legal Disputes” and based on the legal ground that “Data processing is mandatory for the establishment, exercise or protection of a right“. If your Personal Data is requested by the authorized persons, institutions and organizations, we may transfer your Personal Data to such authorized persons, institutions and organizations, administrative authorities and legal authorities for the purpose of “Providing Information to Authorized Persons, Institutions and Organizations” and based on the legal ground that “It is mandatory for the data controller to fulfil its legal obligations“.
3- Your Claims Regarding Your Personal Data
As the Data Subjects, you may submit your claims within the scope of Article No11 of the Law titled “Rights of the Data Subject” regarding the matters mentioned in Article No (1) and Article No (2) to our Company in the following ways in accordance with the Communiqué on the Procedures and Principles of Application to the Data Controller:

Please note that it is important that your application includes your contact information where we can reach you, your identity information where we can verify your identity and if any, documents supporting your claim, in order to finalize your claims in the healthiest way and within a short period of time.
